Neither you nor I have any idea how much antifreeze you have in there. If it's been boiling out and first you added water in an unknown quantity, and then you added 50-50 antifreeze in an unknown quantity, the only thing that can be stated for certain is that the amount of antifreeze you have in there is not correct.
You need to have about 4 and a half quarts of 100% antifreeze in there. The rest needs to be clean water. Alternatively, completely drain the system and pour in about 9 quarts of 50-50 antifreeze. Completely draining the system is not an easy job.
Have you replaced the pressure cap as suggested?

Look for 100,000 mile five-year anti-freeze. You shouldn't mix colors. There are green, orange and gold colored antifreezes on the market. By now, you probably have no idea what was in there before. Best thing to do is get the system fully flushed and properly refilled.

When you go in for the system flush, have them run a pressure check. They will pressurize the system and see if it holds. If it doesn't hold pressure, there's a leak somewhere. With luck, they'll be able to spot it. Like Tech says, you can't do that over the internet.
