Default 2004 Chrysler Pacifica Problem

What's going on? This is actually for my mom. I my sefl have a Dodge Magnum so I'm in the car family as well lol. Here is her situation.

"My fuel gauge simply drops to E; actually below E like it just quits working and the low fuel lamp lights and it tells me low fuel. I know it's not empty, when it acts normal the reading is 1/2 a tank. The gauge reads normal for about 10 seconds then drops to E again. I stopped a couple of times on the way home and it did the same thing each time.
Starting the car, the gauge reads correctly but after maybe 10-15 seconds of driving, it goes back to E. I've noticed this happens when I travel between 20 to 30 or stopped at a light. When I'm on the highway, it doesn't happen."

Anyone ever experience this or have any idea what this could be?

Thanks!!
