Default Codes PO137 and PO171

I have a 97 t&c lx with 3.8L while driving i kept losing power then it would strighten out and drive normally then lose power again until finally it stopped and died and will not restart. Code reader says i have code PO137 (O2 sensor curcuit low volts bank 1 sensor 2) and a code # PO171 (system too lean bank 1) What would cause this power loss and eventually die and no restart? At first i thought i had a problem with the pcm (problely still do for sure now) but everything was working fine until it suddenly lost power, i did notice that at times when the van was cold the wipers would make one wipe across windshield but none after that. i did notice a bad vibration with van while it was losing power the more i tried to accellerate the more vibration there was in it, could this mean the pcm is out? I know it controls the trans shifting. also on my code reader it gave me another code of Po137 but with p/d (p above the d) i have never seen that before on a code reader. Any ideas on this would be appreciated. Thanks

ok I think i found my problem, unknown to me until now my wife tells me that the fuel gauge is way off, it showed little over a 1/4 tank of fuel in it. I remove fuel cap and rock vehicle well could not hear any gas slushing around so i popped hood and bled off the fuel rail i got the pressure coming out but no fuel so i went got fuel put it in and after a few pump primes it started right up and drove normal like it should. So now i have tp pull fuel tank off to see whats going on, i installed new pump last summer but i never knew the gauge was off like that. I also cleared the codes out and drove for about 1/2 hour and they haven't come back on yet but time will tell i guess.
