Default MIL CODE 2097? 2005 T&C ~103K miles

I just got this one while driving down the freeway. Had recently done some routine maintenance. Changed spark plugs/wires, oil, air filter, and PCV valve. Now, the PCV I wound up going with a "non Chrysler" valve because the job was an unexpected PAIN. However I got it in and haven't had any trouble for a few weeks and about 1,000 miles.

This code is showing "EGR" on my reader. Now, I went through all this about 30K miles ago and replaced the EGR valve due to running problems. The vehicle is running OK now, but on a side note I dumped about a gallon of regular green antifreeze in. Reason was I wasn't sure what the code was and had a small trip to go on. I noticed it was low in the reservoir on coolant so thought maybe that was a problem. Now, I've never flushed the coolant on this thing or done anything with it. So, will I now need to flush the coolant? I put in standard green , and it looks like it was red from the factory (or the last flush which I cannot remember)

Also, what can I do to fix this code? Could the EGR valve be bad again?!

Well it threw a code again. This time two codes. I think the 2097, and I think 1028, something with O2Sensor/O2Heater in it. Is this just a sign a need a new O2 sensor? If so, which one?

I believe it's p2038 and p1097. Now, this code literally comes and goes about every 100 miles driven. I picked up a downstream O2 sensor but I'm really not sure what's throwing these codes?
